Special Diet Infant Formulas


These infant formulas are made for kids with medical issues. 
Give the parent/guardian a Medical Statment for Meal Accommodations (see the Forms section). 
The doctor must document a disability or issue affecting a major bodily function

Preemie formulas have extra calories for growth.
  • Examples include: Enfamil EnfaCare, Similac Neosure,
Hydrolyzed protein formulas - some proteins are broken down.
  • Examples include: Comforts Hypoallergenic, Enfamil Nutramigen, Mead Johnson Pregestimil, Nestlé Extensive HA, Nutricia Pepticate, Similac Alimentum, Up & Up Hypoallergenic
Amino acid formulas - all proteins are broken down.
  • Examples include: Abbott EleCare, Nutricia Neocate, PurAmino
Metabolic formulas - Formulas for kids with metabolic disorders
  • Examples include: Phenyl free, MSUD, etc.
Low-Mineral Formulas (low-iron for low kidney function, etc.) 
  • Similac PM 60/40
Formula after 13 months of age
  • If a child needs formula after 13 months of age, we will need a new physician statement to show there is a medical need to continue infant formula. 
  • A Physician Statement is required for ANY "Toddler" formulas for kids over 1 year old. This includes Nestle Nido, Pediasure, etc. 

Formulas made OUTSIDE the U.S. that are NOT FDA-APPROVED are NOT creditable.
